Cited finding
During a review of children's records, it was found that three children's emergency forms are missing information. Two emergency forms are missing the child's address and one emergency form does not have any emergency contacts listed. Please notify the respective parents of the missing information so that they may add it to their child's form. Please have the parent initial and date the form to indicate the update. Send a copy of the updated emergency forms with the added information to OCC for the correction of this non-compliance.
