Cited finding
During a review of a sampling of children's files, it was found that a 5 year old boy and a 5 year old girl both need another MMR vaccination. In addition, a 2 year old boy did not have any immunizations in his file. The Center Director contacted all three parents during the course of the inspection. The Center was able to obtain updated immunization records for both of the 5 year old children. Therefore, that portion of the non-compliance is now corrected. The Center still needs to provide a copy of the 2 year old boy's age-appropriate immunization record for the correction of this non-compliance.
