One child who meets the birthdate requirement for blood lead testing was missing blood test results during inspection today. Please obtain from parent and add to child's file. Please send copy to LS.

Two emergency forms need health care provider information added; one of those forms needs parent signature. Please obtain from parents and add to children's files. Please send copies to Licensing Specialist.
When LS was checking the bathroom, the water temperature read between 130-132 degrees F. Water temperature is not to exceed 120 degrees F. Provider should adjust the temperature of the hot water heater so the water temperature does not exceed 120 degrees F. Please send written plan of correction to LS.
When Licensing Specialist arrived at the provider's home, LS observed a 9 month old infant asleep in an inclined infant toddler rocking chair, which was Fisher Price brand. The chair is not an approved sleep source and the child should have been moved from this chair to an approved sleep source when she fell asleep. The child's playpen was moved into the living room by the provider while LS was present. The child was removed from the chair while LS was present. The infant stayed awake and provider held her for a while and then got on the floor with her to play. Provider is reminded that children are never to sleep in unapproved sleep sources and, if they fall asleep in one, they should be moved immediately to an approved sleep source. Licensing Specialist will check the CPSC recall list regarding the infant toddler rocker.
When checking the bathroom, Licensing Specialist observed the cabinet under the sink in the bathroom to be unlatched. This cabinet contained toothpaste and body lotion, which are considered potential hazards to children, and should be inaccessible to them. Provider should relocate these items to an area that is inaccessible to children or latch this cabinet to make it inaccessible. Please send photo and written plan of correction to LS.
Provider has been using the 'den' room for infant sleeping. This room has previously been approved for use, but is not currently approved by the fire marshal's office. LS observed the room during inspection and has no objection to approving the room for use again once fire marshal's office has given their approval. In the meantime, please use one of the other rooms approved for sleep for this child. Currently, rooms approved for sleeping are the upstairs living room, the downstairs living room and the other two bedrooms. LS strongly recommends using one of the rooms upstairs as opposed to having children on both levels. LS will request a new space fire inspection. Please send written plan of correction to LS.
LS observed a hole in the mesh netting in one of the portable playpens provider is using for infant sleep. Provider should repair or replace playpen. Provider stated she has other playpens and she will replace the one with the hole in it. Please send photo of playpen once replaced or repaired.
Licensing Specialist observed toothpaste and body lotion in the bathroom sink cabinet. These items are potentially hazardous to children. Provider relocated the items during the inspection. This is corrected.
LS observed a mini trampoline on the driveway during inspection today. Children are not allowed to have access to or use any type of trampoline. Please remove the trampoline from the outdoor play area and make sure that it is inaccessible to all child care children. Please send written plan of correction.
