One 4 year old preschool child did not have blood lead test results on file during inspection today. Please obtain a copy of results from parents and add to file. Please send copy to LS.

One employee was added in April 2024, but LS was not notified in writing of the addition until May 2024. Program is reminded all new employees are to be reported to OCC within 5 business days.
One employee left employment in April 2024, but written notification was not provided to the office until May 2024. Program is reminded the operator shall provide written notification to OCC about the ending of employment of an individual within 5 business days.
One employee began employment without being fully cleared by the Licensing Specialist. LS had fingerprint results, but no notarized release form to conduct a clearance for child and adult abuse. Once LS was made aware of the new employee, the program forwarded a release form almost immediately and LS was able to get the employee fully cleared. Program is reminded no new employees may start work until LS has fully cleared them and the school has received an email from the LS stating the new employee is fully cleared to begin work. This has been corrected.
Medication administration forms are good for 1 year. One child has a diabetes management plan on file at the facility, but is from the 2023-2023 school year. Please obtain an updated diabetes management plan, which includes all medications at the school, from the child's parent and add to file. Please send a copy to LS.
Today was an early dismissal day for the school, so there were children present in Educare who ate lunch. The program did not furnish milk to the children for their meal. Since early dismissal days are the only times the Educare children eat lunch at the program and those days are very few during the school year, LS recommends having some shelf stable fluid milk on hand to offer the children. Director stated she will purchase milk and will offer it to the children on the next early dismissal day.
Operator did not notify the Office of the Agent-Educare Substitute leaving employment. OCC 1203A was completed by Licensing Specialist today at inspection. A 1203 form was given to the Acting Principal to complete and submit to OCC. Please send to Licensing Specialist once completed.
