Dust from the vents in the baby room was found on the white mattress pad during time of inspection and dust noticed on the blades of the fan.
It was stated that ventilation has dust falling from the air vents in the baby room and the babies are putting it in their mouths.
Adequate supervision of children at all times is not being performed due to staff using their cellphones
