In the storage closet in Room 1 LS observed materials in disarray and flat black mouse traps with peanut butter. In Room 5 Infant B LS observed a hole in the wall in the corner of the classroom where the changing table meets the wall. LS observed in Room 5 Infant C mouse droppings inside the tracks of the changing table. Facility needs to send a letter of corrections and photographic evidence to LS identifying how this has been corrected.
LS observed toilets in Room 7 and 3 with urine covered seats and dirty bowls. Teachers cleaned these areas while LS was present.
In Room 5 Infant B LS observed a hole in the wall in the corner of the classroom where the changing table meets the wall. LS observed in Room 5 Infant C mouse droppings inside the tracks of the changing table. In the storage closet in Room 1 LS observed materials in disarray and flat black mouse traps with peanut butter. LS observed in Room 5 Infant B a dust covered return vent in the ceiling. LS observed In several classrooms the paper towel and soap dispensers are dirty. LS observed a broken (cracked) chair in Room 4. LS observed in Room 3 a bookshelf that is in disrepair. LS observed cots in room 1 that are fraying and in disrepair. Facility needs to send photographic evidence to LS that these items have been repaired/corrected.
LS observed the trash can in room 5 infant B did not have a disposable liner. Facility needs to send photographic evidence and a letter of correction to the LS informing how this has been corrected.
LS observed a cracked plastic portable storage container and a cardboard box on the side playground outside Room 1/2. Facility needs to send photographic evidence identifying how this has been corrected.
LS observed two tricycles that are missing pedals and a toy truck that is missing front wheels. Facility needs to send a letter of correction informing how this has been corrected.
In room 3 LS observed 6 children present and only 5 children signed in. In room 4 LS observed 5 children present and 6 signed in. Facility needs to send LS a letter of corrections identifying that safety and security policies and procedures have been reviewed with all staff to ensure the whereabouts of children at all times.
Licensing Specialist observed thresholds loose, ceiling tiles broken or displaced, part of the overhang outside is in disrepair separating from the building doors and molding in need of painting throughout, no door hands on some exterior doors, dirty window and door tracks.
Licensing Specialist observed several bathrooms throughout the center where the mechanical ventilation for the bathrooms were clogged and inoperable covered with dust. Clean and repair as necessary.
Licensing Specialist observed toilet seats in several of the children's bathrooms need to be replaced because that are not smooth and easily sanitized. Bathroom in older three's class has a shelf above the toilet that is not level and unsafe. Vents in bathrooms are clogged with dust and not providing adequate ventilation throughout.
Licensing Specialist observed ceiling tiles with cobwebs, tape, and dirt throughout the building. All vents were dust covered. We recommended the center get deep cleaned. Bathroom floor in the two year old room contained a bug trap and observed dirt and crumbs on the floor.
Licensing Specialist observed green mold like substance on the ground, the walls and the fence on the playgrounds. Spider webs and cobwebs under the overhang and on the windows, trash and broken materials, large gaps in the fence where it meets the ground, splintered retaining walls, gaps in the retaining walls, sticker plants on side playground, ladder accessible, broken table, and broken water play table. Classroom doors to the outside need handles. Doors were propped open.
Licensing Specialist observed two high chairs one in infant B and one in infant C that are missing parts of the restraint.
Licensing Specialist observed a staff members purse on the floor in older three's classroom, broken shelf taped in older three's classroom, and medication stored in plastic container under crib in infant B.
