LS observed children's files. Multiple enrolled children with medications did not have parent signature on the medication administration for to grant facility permission to administer medication.
LS observed that multiple staff medical evaluation forms were incomplete.
LS observed visible build up in multiple microwaves throughout the facility. In addition, LS observed visible rust residue on a child toilet in room 6.
LS observed exposed lining throughout multiple areas of the playground that had a reduced presence of mulch. The exposed lining could potentially pose as a tripping hazard.
LS observed multiple pieces of play equipment that were rusted or had chipped paint.
LS observed multiple classrooms did not have a daily schedule posted that contained a timeline of activities on a daily basis.
LS observed disinfectant spray and diaper cream within reach of children in multiple classrooms. Please ensure that all cleaning agents and items that state "keep out of reach of children" are stored in a manner that makes them inaccessible to children.
LS observed a resting infant in a crib with a bib around their neck.
LS observed several items, including a broom, dust pan, step stool, a backpack and a package of diapers, blocking the bathroom sinks and stalls in room 2. These items are a safety and tripping hazard, and it blocks access to the sinks for handwashing. These items were moved at the time of the inspection.
