During health and safety inspection the following was found: many rugs throughout the center need to be replaced due to wear and tear, wet ceilings tiles, missing wood trim from area in preschool classroom and a toilet in an empty toddler room was filthy and needs to be replaced.

Licensor observed an educator leaving one infant room to go get something in the neighboring infant classroom, leaving the room they were assigned to out of ratio.
Areas throughout the center were found in need of painting, Areas were found peeling and from normal wear and tear.
Safe sleep violation found during visit. An infant was observed sleeping in a sleep sack that tucked their hands inside the sack, not allowing their hands to be out and free to move around.

Educators in a preschool classroom, where a child has a [REDACTED] did not know how to administer the child's emergency medication. This was cited during previous licensing visit.
A toilet in a preschool room would not flush.
Two children with chronic medical conditions, requiring emergency medication, were not listed on the required classroom posting.
Random review of 5 children's files found 1 with most of the enrollment packet not dated therefore could not determine if it was current, 1 file with expired enrollment paperwork and no current physical and 1 file with no consent for EI to work with the child at the program.

During health and safety inspection the following was found: overall classrooms were found dirty and unorganized from floors to toilets to counters, etc..., gate to a bathroom is broken, trash on floors in bathrooms, and tops of trash cans filthy, Entire center needs deep clean and ongoing cleaning maintenance schedule.

Random review of 3 staff files found the following: 2 had no current physical, 2 had no evidence of immunity to MMR, 1 had no documented program orientation and observations of staff are not happening at least every other month.

During the visit the preschool classroom had two assistants and no teacher qualified educator in the classroom
When entering a classroom an educator was standing with their back to the children and the door while talking to the educator in the bathroom changing diapers. The educator did not turn around to see who entered the classroom nor was the educator supervising the children behind their back.
An educator was observed having a negative tone and language with the children in their care. They stated things such as: "Because I am the teacher and I'm telling you", repeatedly telling children "No" and "Don't do that" with no explanations.
Educators in a classroom, with a child who has a seizure disorder, were not trained on how to administer the child's Diastat emergency medication. When asking educators in another classroom if there were any children with emergency medication, neither educator was aware if there was anyone and one stated, "there may be in inhaler".
A staff records checklist was not available during the visit. A current checklist has not been maintained.
Review of IHCPs found there was no plan on file for 2 children with chronic medical conditions and 1 child's IHCP was not dated so it could not be determined if it was current or not.
A child's bottle of [REDACTED]expired. Another child's inhaler was out of the prescription box on the bottom of a classroom emergency bag.
Educator in a toddler room was not cleaning and disinfecting properly.

During observations there was no teacher qualified person in one infant room. There were 2 assistants providing coverage.
During inspection of indoor space the following was found: not all bases of toilets clean, dirty refrigerators and area rugs.
Inspection of the playground found the following: a dirty Band-Aid sitting on a table, peeling paint on the chain link fences/gates and hose strewn on ground and not secured.

An educator was observed changing an infant's diaper and they did not discard the changing paper or wash and disinfect the changing surface when done. The educator did not wash their hands or an infant's hands after diaper changing and proceeded to feed the infant their bottle. An educator in a toddler room, when asked what they used on diaper surfaces, stated they used the blue bottle (disinfectant). Surface not being cleaned prior.

Review of 5 children's files found 1 file with no current [REDACTED]
One educator's file did not have current first aid training documentation.
When educators were asked how they warm infant bottles they stated they get hot water from bathroom sink outside of the classroom or from the water cooler in the program entry.
Not all infant bottles and water bottles in other classrooms were labeled for the child they were intended for.
During health and safety inspection the following was found: mops and brooms not secured, in general, equipment (including tables, refrigerators, counters) windows and surfaces not clean and nap mats torn. An infant's bottle was found dripping on the classroom carpet. Educators in toddler classroom storing attendance and Ipad on half wall which poses a potential hazard.
Preschool climbing structure has rust and broken sharp edges on side of stairs.

During review of all staff background record checks for license renewal it was learned two educators have been working in the classroom and are still showing as "Pending" in the BRC system. The Director did not ensure the educators had a completed BRC prior to offering them employment. Director has not ensured timely follow up with educators that have had "Provisionally Approved" BRCs for approximately 1 year.

Bottle Warming
Licensors observed a thermal carafe on the counter in the infant classroom. Educators stated their sink does not have hot water so the carafe is filled with hot water to then use to warm the bottles.
Area with mulch continues to be a hazard for children. The area does not stay filled, causing a lip to the concrete that becomes a tripping hazard. The I/T playground had prickly vines intertwined in the fencing accessible to children and a corner filled with leaves/debris and some trash.
A child's emergency medication was found in a different classroom than the child was in.

Not all water bottles were found labeled for the child they were intended for.
A diaper changing surface had cracks in it, not making it possible to effectively clean and disinfect.
Not all refrigerators had thermometers therefore it could not be determined if they were being maintained between 32-40 degrees.

During health and safety inspection the following was found: toilet brush was in front of toilet leaning against a crate, windows throughout the center are dirty, torn nap mats in multiple classrooms, dirty toilets/sinks/counter spaces, bathroom areas with strong smell of urine, torn blue child's couch, dirty carpets and floors, countertop to child sink is breaking off, educators personal drinks and bags within reach posing potential hazards and dirty refrigerators. Overall the general cleanliness of the program is not being maintained.
