First aid kit was missing tweezers. Please add tweezers and confirm once completed.
There was chipping paint observed on the side of the house (lower and upper levels), on the trim, and shed of the home. Please inspect entire outdoor space and remove all chipping/peeling paint; confirm once completed and a follow up visit will be scheduled.
The exit from the basement was blocked with random items. Please clear this space and upload a picture once completed.
Licensor walked through the indoor space and observed the following hazards: There were loose plastic bags throughout the space that the children had access to. In a drawer, there was a bottle of Benadryl and fragrance spray, and in another drawer there were multiple lighters and matches. Please add locks to these drawers or remove items to an inaccessible location. Confirm once completed. There was also a small trampoline located in the play space; all trampolines are forbidden. Please remove trampoline from the area and confirm once completed.
There is one child enrolled that has a peanut allergy. Please post children's allergies and medication, and ensure that it is done so in a confidential manner. Confirm once completed.
There is one child enrolled that has [REDACTED]and there was no IHCP on file. Please obtain IHCP from child's doctor, add to the record, and ensure it is renewed yearly. Confirm once completed.

Progress Reports
Progress reports were out of date for all children. Please update all progress reports are updated and confirm once completed. Also, add a plan on how progress reports will remain up to date moving forward.

There are two cats in the home that are reportedly vaccinated. During this visit, the cats were observed to be on the kitchen counter. Please ensure cats are not on any surface that has contact with the children's food.

This follow-up visit inspection recorded no violations or advisories.
Fire drill was not completed in October or November. Please ensure fire drills are completed and documented monthly, and add how you will ensure this is done.
There were two infants napping at the time of this visit, and both had small toys in their pack and plays. Licensor discussed the safe sleep policy and the need to ensure that sheets are fitted, no blankets, pillows or other hazards be placed in the pack and play of an infant under 12 months. Licensor sent Educator the Safe Sleep Pledge, Safe Sleep Parent Letter and the Safe Sleep Training. Once completed, please provide Licensor with the certificate.

Educator had an online only CPR and First Aid certificate. Please complete a new course which contains an in person component, and provide certificate once completed.
The refrigerator was broken. Once fixed, please obtain and provide internal temperature.
Licensor walked through indoor space and the following hazards were observed: FIRST FLOOR- *Kitchen- There were alcohol bottles accessible on the counter top. Please remove all alcohol bottles and make them inaccessible to children. *Dining Room- There were scissors located in a drawer and cat food on a counter top. Please move items to make them inaccessible to children. BASEMENT- There were light bulbs stored in accessible drawers. Please make these items inaccessible. There were stacks of toys piled on top of a coffee table. Please organize all toys and make sure there are no items stacked on top of one another to avoid injury. There was a television that was not anchored. Please anchor to avoid a tipping hazard. *Living Room- There was a television that was not anchored. Please anchor to avoid a tipping hazard. *Dining Room-There were uncovered electrical outlets; please cover all outlets. Licensor walked through outdoor space and the following hazards were observed: There was excessive dog feces in the back yard and some in the front yard. Please ensure this is cleaned to ensure children don't touch or consume it.

Licensor reviewed two children's and they were missing progress reports. Educator reports that she gives parents the progress reports. Please ensure that a copy of progress reports are kept in child's files moving forward.
CPR/First Aid Certification
CPR Expiration Date: 4/21 First Aid Expiration Date: 4/21 Educator was not aware that certificates could be renewed during the pandemic. Licensor discussed with Educator that she cannot care for children with expired CPR and First Aid. Please renew your certification as soon as possible and send certificates to Licensor.
The first aid kit was missing a gauze roll. Please add this to the kit and confirm once completed.

Licensor reviewed children's records and there were two missing up to date physicals and one missing an annual update form. Please review all records and confirm once all were up to date.
Evacuation was not completed in March. Please ensure evacuations are completed monthly; confirm which reminders will be used to ensure it is done.
There were cleaning sprays accessible to children. Please remove all cleaning products and confirm once completed.
Licensor walked through licensed space and the following hazards observed: Dining Room- There were plastic bags accessible to children. Please make inaccessible to children and confirm once completed. Kitchen- There were toys scattered in the kitchen eating area, which posed a tripping hazard for children. Please ensure the space is always clear for children to walk; upload pictures once completed. Basement: There were numerous toys and other materials scattered on the floor. As mentioned above, it was a tripping hazard. Please clean this area and upload pictures once completed. The outdoor space is located in the backyard. Licensor observed a fire pit that was uncovered. Please cover and confirm once completed.
Educator reports that she does not always use a disposable layer when diapering. Please ensure disposable layers are used for every diaper change; confirm what will be used for this layer.

Licensor walked through indoor space and observed the following hazard: Overall, the home appeared cluttered and counter/table tops were covered in random items. Educator reports that this was due to her broken fridge. It should be noted that this has been cited in previous visits. In the dining room, there was a vacuum cleaner, with it's power cord accessible. Please tie this up and remove from child care space. There was also a bag full of plastic bags accessible, which presents a suffocation hazard. Please make this inaccessible to child care children. Confirm once completed.
Licensor walked through the outdoor space and the following hazards were observed: There were loose hoses, which should be tied up; please do so. There was a pool filled with water, which presents a drowning hazard; please empty the pool and ensure pools are emptied daily. There was also hot tub that was broken and taken apart; please remove this equipment or make inaccessible to children. Confirm once completed.
