The first aid kit was missing children's emergency authorization forms for children. Please ensure forms are added to the first aid kit and confirm once completed.
Educator did not have training certificates available for 2023 and 2025 as Educator reported she did not have them accessible during this visit. Also, there were not 10 hours of diverse learners trainings. Please provide all training certificates and if none were completed in that time, please complete additional trainings that meet 30 hour requirement and 10 of which are in diverse learners. Please upload certificates once completed.

Licensor walked through indoor and outdoor space, and the following hazards were observed: In the kitchen, there was a knife block on the counter. Please ensure this is made inaccessible during child care hours. In the living room, there is a fireplace which has a hearth surrounding it. Please pad this hearth to ensure there are no injuries. There were stairs that did not have a gate. Please ensure these stairs are always gated during child care hours. Please send pictures once completed. In the outdoor space, there are two plastic slides that exceed 18 inches. Please remove these slides from the space or add a fall zone.
