Educator was not present and assistant was running the program. There were four children present (ages 6 months, 8 months, 15 months, 16 months) and assistant was alone. One child was sent home to ensure program was within ratio. Please ensure that program is always within ratio and confirm adult-child ratio is understood.

Children's records were incomplete as forms were located in different folders. Due to this, enrollment packets were not assessed. Some of the children's records were also missing up to date lead tests. Please utilize the Children's Record Checklist to ensure all records are up to date and organize records to ensure they are easier to manage; confirm once completed.
Emergency number posting was missing the program's address and phone number. Please add and confirm once completed.
The only hazard observed in the program was an exposed nail on the changing table. Please make this hazard inaccessible and confirm how this was done moving forward. The outdoor space is a fenced in area of the background and there were no hazards observed.

Licensor reviewed children's record and several were incomplete; missing medical documents and signatures. Please review records and ensure they all have enrollment packets, physical/medical documents, lead tests, and any additional paperwork if children have any life saving medications. Please confirm once completed.
Children's diapering materials were all together in a bin. Please separate all the children's belongings in separate containers that have children's names. Also, Educator does not use a disposable layer between each child. Please ensure a disposable layer is used and identify what that will be moving forward.

Educator did not have any training certificates on file; if there are any certificates on the computer, please email to Licensor. If not, please take 20 hours of training and email certificates to Licensor.

Licensor reviewed records and most were missing up to date permissions and annual update forms. Please review records and ensure all forms are updated (annual update form was emailed to Educator); confirm once completed. Also, add plan on how Educator will ensure records are up to date moving forward.
The children's toothbrushes were not stored in a sanitary manner (they were all laying together in a small bucket). Please create a new system where the toothbrushes are stored sanitarily and clarify what that will be moving forward.
First Aid Kit
Licensor reviewed children's records and there were several documents missing, such as medical forms and enrollment packet pages. Please review all records and ensure they are complete. Please note that this must include up to date physicals, signed transportation, enrollment forms, immunization records, progress reports, parental permissions, etc. Confirm once completed.
Educator hasn’t taken any trainings. Please complete 10 hours of trainings and provide certificate to Licensor.

Educator had no staff records for assistant. Please ensure a record for assistant is done, which should include the medical letter, trainings, CPR and First Aid, and attendance; confirm once completed.
Refrigerator thermometer was broken. Please add new thermometer and upload picture once completed.
First aid kit was missing instant ice packs. A child was witnessed to bump his head during this visit and Educator used a common ice pack from the freezer. Please ensure an instant one is added to the first aid kit and it is used once per child.

The first aid kit is missing emergency card form/authorization for emergency treatment. Please add this form for all children and confirm once completed.
Children were eating appropriately, however hand washing was not completed after. Please ensure handwashing is completed after eating.
