Licensor walked through outdoor space and observed the following hazards: There was a kiddy pool and water table that were not emptied. Please empty all pools/water sources after each use; confirm once completed. There was also a hose that was loose and accessible. Please tie hose up and confirm once completed.
Educator uses an app for attendance, and does not keep a written copy. Please ensure attendance is documented on paper, which includes the child's name, date, arrival time and departure time; please confirm this will occur moving forward.

Licensor walked through licensed, indoor space and the following hazards were observed: There were dumbbells located in the living room. Throughout the home, there were toys strewn about, power tools accessible, and a trash can out. In a kitchen drawer, there were screwdrivers that were accessible. Lastly, a child's rags were strewn about the home, which all children had access to and were touching, which is a sanitary issue. Please ensure all items and hazards are removed or locked. Furthermore, add a plan on how the home will be maintained safe and clear of hazards moving forward. Upload pictures once completed. Licensor walked through outdoor space (backyard) and there were no hazards observed.

The play room door is missing an exit sign. Please add sign and confirm once completed.
There was no evidence of curriculum, structure or routine within the program. Please develop curriculum materials and a written daily routine, and upload once completed.
Refrigerator Temperature
There were items blocking the exit from the play room, therefore compromising exit safety. Please note this was cited in Educator's previous visit. Please clear this exit and upload a picture. Furthermore, ensure all exits are always accessible and safe.
Licensor walked through licensed space and the following hazards were observed: There were multiple trash cans without covers throughout the space. Please ensure there are only covered trash cans in the licensed space. There were dirty dishes accessible throughout the program. Please ensure there are no dirty dishes accessible to child care children. There were other hazardous objects throughout the program such as lotions, scissors, and power cords. Please remove all these items and make sure the licensed space is free of hazards. Lastly, the licensed space was overall unorganized and unkept. This has been cited in Educator's previous visits. Please clean the home and upload pictures once completed. Ensure space is clean and free of hazards at all times.

There were electric fans accessible in the licensed space. Please make these inaccessible to child care children.
Licensor walked through licensed space and the following hazards were observed: *KITCHEN- There were knives and cleaners accessible on the counter. *PLAY ROOM- No hazards. *BEDROOM 1- There was laundry, trash, and power tools on the floor. *BEDROOM 2- There was an uncovered trash can. *LIVING ROOM-There were several random items (books, toys, trash, etc.) strewn about the room. *BATHROOM- There was an uncovered trash can. Overall, the home was unkempt and unorganized. As mentioned, there were items strewn about the home that posed as a tripping hazard to children. Please clean the entire space, remove all hazards mentioned in rooms, and upload pictures once completed. There were no observable hazards in the outdoor space (fenced in back yard).
Children's emergency contact information was missing from first aid kit. Please add and confirm once completed.
Educator was asked to provide documents via email, but Licensor never received them. Please upload these documents or email to Licensor.
