Through interview it was learned that the Program has not done or offered parent conferences
Review of the staff records checklist revealed that one Educator has a suitability expiration date of 10/6/25 which is now expired.
The program has not completed progress reports for school age children once per year as required by EEC.
On the day of the visit files were maintained in the reception office in a cabinet that was not locked to prevent unauthorized access.
Licensor observed two children in the snack area with an Assistant Group Leader. One of the children left the area and went out the back door to the back hallway, that leads to the back exit door. The child was unsupervised Supervision at the time.
On the day of the visit the attendance records in two rooms did not accurately reflect the number of children present.

On the day of the visit the program had a medication that was listed in US Drug Enforcement Administration (DEA) Schedule II-V that were not kept in a secured and lock place.
The program did not have parental consent for the administration of medications, required by an IHCP,for at least 3 children.
Evacuation procedures, including the process for evacuation non-mobile toddlers, were not posed in the new preschool or toddler rooms
The program has been open since October 2024. The program has only completed one evacuation drill, which was conducted and documented on 2/18/25. EEC regulations require that evacuation drills to be conducted every months with all groups of children.
[redacted] was unable to find the Group Leader’s file so EEC licensor was unable to determine that the [redacted] is qualified for their position.
On the day of the visit [redacted] did not have evidence they are conducting evacuation drills every month as required by EEC Regulations.
The program did not have parental consent on file, to administer two different emergency medications, required for a child with two different chronic medical conditions.

The program did not have or maintain a snack menu for the month of July as required by EEC.
A staff person’s pocketbook was stored within reach of children on the day of the visit.
On the day of the visit there was a spray bottle of multi surface cleaner stored within reach of children.
The gate to the kitchen area where there is a toaster oven, air fryer, coffee maker and microwave was not secured allowing childen easy access to the areas.

EEC Licensor requested documentation of Staff meetings on two occasions but the information was not provided. EEC Licensor was unable to assess compliance with regulations related to staff meetings.
A review of staff records revealed that some filed did not include evidence of Reference checks, Documentation of interviews, evidence of PQR registration, and evidence of two doses of MMR vaccine.
The program is requesting the addition of toddle and preschool rooms. Inspection of the rooms revealed the program did not have rest materials including cots for the children.
On the day of the visit the attendance record did not include a child that was in care at the time of the visit.
Inspection of the new Toddler and Preschool rooms did not have developmentally appropriate tables and chairs for the children.
The fenced in grassy area used for out door play did not have natural shade or shade structures on the day of the visit.
On the day of the visit the program did not have a posting indicating the location of the Health Care Policy.
The posted Emergnecy procedures did not include the method used for educators to ensured that all children are safe evacuated.
Inspection of the first aid kit in the office revealed the kit included the following items: pain relief tablets, antibacterial ointment, antiseptic wipes. The program did not have parental permission for the use of these items.
Review of Children’s files revealed the program had at least 4 IHCP for children, with medical conditions, that were not completed. The IHCPs were missing information including the description of the medical condition, the symptoms, the names of staff trained to administer treatment, and the name and telephone number of the physicians.
The program did not have a weekly snack menus in place and there were not previous menus prepared.
On the day of the visit Educators failed to clean and sanitizing tables surfaces prior to serving children snack.

On the day of the visit [redacted] did not have evidence they are conducting monthly staff meeting for 2 hours each month as required by EEC.
The program did not have a posting with childrens allergies/medical information.
A review of staff records revealed the following: 1) Two Educators did not have evidence of completing the EEC Essential Trainings. 2) [redacted] was not able to find the file for the [redacted]. 3) One Educators file did not include evidence of a current physical, MMR vaccines, Staff Form, or Verification of work experience.
On the day of the visit [redacted] left the room leaving [redacted] alone to supervise a group of school age children.
On the day of the visit it was revealed that one vehicles used to transport children did not have documentation of a second post trip inspection conducted by a 2nd reviewer, as required by EEC Policy.
One driver’s 7-D license was not on file on the day of the visit. EEC Licensor was not able to assess if the driver holds current 7-D certification.
The program did not have evacuation procedures posted near the front and back exits.
A child with two different chronic medical conditions did not have an IHCP for one of the conditions. The IHCP for the other condition did not include the parent signature.
The program did not have a supply of substitutes meals or snacks in case a child attends without it during no school days.
