The program did not have documentation of progress reports on file.
Multiple maintenance and safety issues were observed at the time of the visit that require repair or attention. 1- Floors appeared to need cleaning at the time of the visit. 2- A shoe rack was broken. 3- One classroom had small puzzle pieces scattered on the floor and appeared to need cleaning and organization. 4- A window frame in one room had peeling paint, and the trim around the border appeared worn. 5- Ceiling tiles in one bathroom showed water leak stains. 6- Cleaning materials were accessible and must be kept out of children’s reach. 7- Peeling/chipped paint and broken plaster were observed around a sink in one bathroom. 8- Floor tiles had visible cracks and separation along some seams. 9- One bathroom had broken trim along the wall. 10- A sofa showed visible stains and wear. 11- Broken floor tiles were observed in one area. 12- Loose wood boards and other items were stored leaning against a wall near an exit area and were accessible to children. 13- A ceiling access opening was exposed. 14- During the visit, a staff member’s bag was found exposed and within children’s reach.

1- Multiple children had expired IHCP and medication consent forms. 2- Multiple children with medical conditions did not have an IHCP and medication consent form on file. 3-The program must review children's files to identify any children with health conditions that require an IHCP. 4- The allergies and medications list must be updated and posted.
Cleaning materials and tools were accessible to children.
1- There are broken ceiling tiles throughout the facility, with some tiles showing visible water stains from leaks. 2- Furniture and Equipment Wear: multiple pieces of furniture and equipment show visible signs of wear and tear, including: -Upholstered Sofa: The sofa has visible tears in the upholstery. -Table: The table has areas where the paint is scraped, indicating significant wear. -There was a damaged office chair in the child care space with noticeable tears or rips in the fabric. 3-There were potential hazards stored near the exit accessible to children. 4- Some cabinets in the kitchen area and one of the playrooms are in need of repair.

The attendance log does not have two signatures confirming that the vehicle has been inspected at the end of each trip to verify no children were left behind. Licensee reported that he did not know this had to be done.

On the day of the visit it was reported that one driver without a 7-D license was transporting children in a vehicle not registered as a 7-D vehicle.
From a review of medications maintained at the Program it was revealed that not all medications required for children were readily available.

There was no evidence that monthly evacuation drills had been conducted
Some medications were expired and some children were missing medications.
The program did not have all the emergency medications. The Licensee reported it has been challenging working with some parents to obtain the necessary medications after multiple requests.

The first aid kit was missing supplies. The licensee must maintain adequate first aid supplies, including, but not limited to: adhesive tape, band aids, gauze pads, gauze roller bandage, disposable non-latex gloves, instant cold pack, scissors, tweezers, thermometer, and CPR mouth guard.

The program must prepare and make available to parents on request written menus, including snacks, for each week; Previous menus must be kept on file for one year.
The program must maintain as part of a child's record, an individual health care plan for each child with a chronic medical condition, which has been diagnosed by a licensed health care practitioner. The plan must describe the chronic condition, its symptoms, any medical treatment that may be necessary while the child is in care, the potential side effects of that treatment, and the potential consequences to the child's health if the treatment is not administered.
IHCP must be signed by doctor and parent. Individual Health Care Plans The licensee must maintain as part of a child's record, an individual health care plan for each child with a chronic medical condition, which has been diagnosed by a licensed health care practitioner. There are children enrolled in the program who have allergies and medication but a list was not posted.
List of emergency numbers did not include the program's address and phone number.
Procedure was not posted by every exit.
Did not have a fire drill log. Reported that they have done a couple of fire drills.
Several items were missing from the first aid kit: scissors, tweezers, tape, CPR mask and instant ice pack. There were other items in the first aid cabinet like Neosporin and hydrogen peroxide, that should not be used for first aid for children.
On the day of the visit there was no first aid kit in one vehicle.
From a review of a sample of children’s records it was revealed that not all records requiring such had individual health care plans that were accurately and or entirely completed signed and dated.
On the day of the visit at least one transportation driver did not have a passenger attendance log and evidence that the vehicle was inspected at the end of the route.
