The following was observed to be accessible to children: -In TODDLER 1, the short wooden bookshelf and the plastic kitchen were not secure and tippable, there was a rip on the top of the changing pad and the top of the changing table was dirty as indicated by crumbs, dust, and paper trash. -In PRESCHOOL 2's and 3's, the plastic kitchen was not secure and tippable.
Evacuation procedures posted at each exit
The following was observed to be accessible to children: -In the PRESCHOOL OUTDOOR SPACE, two pieces of fabric were exposed from under the rubber mulch and presented as a tripping hazard.

A review of three Individual Health Care Plans indicated that there was no evidence of a current parental medication consent.

Upon arrival, one staff member assigned to supervise six Toddlers left the second staff member alone to supervise the children for approximately one minute to answer the door.

An (REDACTED) child was observed sleeping in a crib with a stuffed animal. Educators must ensure that cribs, portacribs, playpens or bassinets used for sleeping infants under the age of 12 months do not contain pillows, comforters, stuffed animals or other soft, padded materials.
The location of the first aid kit was not posted in the Older Toddler, Young Toddler, and Pre-K classrooms.

Medication
The Infant A and Infant B classrooms were combined on one attendance form. Each classroom must have their own daily attendance record indicating each childs attendance, including arrival and departure times. The attendance was not accurate in the Infant classrooms. There were six children in attendance. However, there were only five children signed into the classroom.
Two children’s Individual Health Care Plans did not include all of the required information.
Building Inspection and Fire Inspection - R Apr 2025
Four children’s toothbrushes were not labeled with their name in the Older Toddler and Preschool classrooms. Children must use individual, labeled toothbrushes which must be stored in a safe and sanitary manner open to the air without touching each other.
One child’s(REDACTED) was not stored in the original package with the (REDACTED) label affixed.
During the visit, the following was noted: The sink in the Preschool classroom is not in working order. An electrical outlet in the Infant B classroom did not include a safety cover to prevent children from accessing the receptacle. Plastic bags, a light bulb, plunger and toilet brush were accessible to the children in the 2nd floor bathroom.

During the visit, two staff files were reviewed. One file did not include documentation of first aid training. One file did not include documentation of the EEC Essential trainings.
Postings
One child’s sippy cup was not labeled in the Infant A classroom. One pacifier and one bottle were not labeled with the child’s name in the Infant B classroom. Personal items intended for individual use by children, including but not limited to bottles, pacifiers, toothbrushes and sleeping materials, must be labeled with the name of the child for whom they are intended.
