In the yellow room, the electrical powerstrip must be placed out of reach of children. There is rust on the stall in the children's bathroom.
The four seat seesaw was placed on the asphalt and must be in an area with an adequate depth of impact absorbing material under and around the fall zones.

At the time of the visit, two classrooms were marked in on the same classroom attendance. Each classroom must have their own attendance showing who is present at any given point in the day.
The location of the first aid kit was not posted in the One-Year-old and Two-Year-Old classrooms.
One child's Individual Health Care Plan did not include all of the required information. One child's Individual Health Care Plan expired on 3/18/2022. One child with an (REDACTED) on the premises did not have an Individual Health Care Plan.
