The following was observed outdoors: acorns were on the ground throughout the Toddler play area and accessible to the children. On the Preschool playground, there was an opening between the chain link fence and the chain link gate that posed an entrapment hazard.

An educator was supervising 8 children in the Young Preschool classroom. At that time, the reading loft was open. The wooden loft is approximately four feet high with a ladder to access it. One child was allowed to use the loft at a time. However, the educator was unable to appropriately supervise the children using the loft and assist them when climbing down the ladder because she was helping other children in the group as well. Licensees and educators must use good judgment at all times and must consider the following factors when determining the appropriate level of supervision: the environment and its impact upon the educator's ability to see and/ or hear children; and the nature of the activity and the materials and equipment used.

The lower cabinets located in the Preschool bathroom did not include safety locks. The cabinets included Lysol wipes and adult vinyl gloves. Educators must ensure that all hazardous objects, including but not limited to matches, lighters, toxic materials, sharp objects, plastic bags, and purses, are locked or inaccessible to children.
Two children's Individual Health Care Plans (IHCP) did not include all of the required information. One child's IHCP did not include the name of educators that received training addressing the medical condition and the name of the person who trained the educator. The second child's IHCP did not include the name of the person who trained the educator.

Barrier to kitchen
The following hazards were observed outdoors and were accessible to children: Bolts from the chain-link fencing (including gates) and the knobs on the shed locks were protruded inward towards the playground. Also, there was unknown thorny vegetation growing through the chain-link fence. Tree roots and playground border timbers were protruding upward, causing a tripping hazard. In addition, the plastic riding ramps did not have the required six feet of fall zone (children could potentially ride into the wood timbers around the climbing area or fall off the side into another child on an adjacent ramp). The merry-go-round, purple dinosaur, pink rocking horse, seesaw, and two slide climbers did not have the required six feet of fall zone (the fall zone was approximately four feet). Two playhouses needed cleaning; the blue and white one had sticks and debris inside. A window screen was torn. The bottom portion of the white exterior trim located near the infant rear exit was splintering and needed repair. A moldy area approximately one foot by three feet was on the vinyl siding of the building and appeared caused by a leaking window air conditioning. The back of the air conditioning unit was exposed in a lower-level window. The white exterior trim (with a sharp edge) near the toddler exit was not properly affixed to the building exterior.

There were two children in the Young Preschool classroom with (REDACTED). During the visit, EEC learned that the children's (REDACTED) are not taken with them to the playground. Emergency medications such as (REDACTED) must be immediately available for use if needed.
The following hazards were observed outside and were accessible to children: The knobs on the shed locks were protruding inward towards the playground, previously cited on 6/11/21. Two of the four riding ramps did not have the required six-foot fall zone, previously noted on 6/11/21. A granite step with sharp corners was installed outside the infant classroom rear exit. This area is used for riding toys. There were exposed nails on a wooden playhouse. The bottom portion of a red exterior wooden gate near the infant rear exit was splintering and needed repair.
In the Toddler 1 classroom, one child's (REDACTED) was not posted in a manner that protects the child's privacy. This was previously cited on 6/11/2021.
During the visit, three children's files were reviewed. One file did not include evidence of a lead test.

The following hazards were observed indoors and were accessible to children: Toddler one room: A plastic bag with a box inside labeled "Tea Party" was located on a table accessible to the children. Preschool room: There was tape residue on the windows, and one ceiling tile is stained. Hallway: Twenty-nine (128 oz.) paint bottles were stored on a half wall by the rear exit not secured.
During the visit, ant hills were observed throughout the playground.
Evacuation procedures were not posted next to the front entrance.
In the Toddler 1 classroom, mosquito repellent and sunscreen were located in a child's cubbie, accessible to the children.
There was not an adequate depth of impact-absorbing material under the swings in the preschool play area. There was not a sufficient depth of impact-absorbing material under and around the merry-go-round, seesaw, and slide climber in the toddler play area as the grass was observed growing through the sand.
The door to the kitchen was left open, leaving the area accessible to the children. The kitchen included cleaning chemicals in the bottom cabinets, knives in the drawers, and a vacuum and mop.
