A review of three Staff Records at the program indicated the following: -Three staff records did not contain evidence of bi-monthly observations as required.
The program's Building and Fire Inspection Certificates expired 3/16/24. Following the visit, the Licensee stated that the building and fire inspections were completed later in that same day (4/9/24) and the program is awaiting the certificates.
The Staff Schedule posted in the program is not current and needs to be updated to reflect recent staffing changes.
