All of the progress reports were reviewed, one child needs a completed progress report.
All of the children’s records were reviewed. One child needs an annual update form and one child needs proof of lead testing.
The educator’s PQR#9775269 expired on 06/16/25.
One of the drawers in the kitchen was not locked allowing access to sharp knives and a bottle opener.

All of the children’s records were reviewed. One child did not have a medical physical with their immunizations and lead testing and date of admission. Two children did not have their lead testing.
The children were observed to eat lunch without washing their hands prior.
In the kitchen, a drawer had a lock but was not functioning allowing access to batteries, gorilla glue and tools. A grater was also accessible in another cabinet. In the bathroom under the sink, their were several cleaning supplies accessible.
Provider’s emergency preparedness plan did not include the section of what they would do in the event a child wen’t missing from their program.
Provider is required to have 10 hours of ongoing professional development. Provider had two hours completed.
