The educator was observed to change the child’s diaper on top of a surface on the couch. The educator does not have a diapering mat. The educator was not observed to wash their hands or the child’s hands after diapering.
In the bathroom, under the sink several cleaning sprays were accessible. The closet door in the playroom was broken.

The educator had several hazards in the desk drawers in the living room including but not limited to the following, cords longer than 6 inches, stapler, knife, screws, batteries and tools.
The stairway leading to the outdoor space does not have a railing or banister installed. The educator does not have a gate at the bottom of the stairwell in the outdoor space.
The carbon monoxide detectors in the basement space were unable to tested. The carbon monoxide detectors on the first floor and second were tested and functioning.
The outdoor play space has a large area of small rocks which pose a chocking hazard.
