A walk through of the outdoor facilities and playground area during the visit indicated the following: -The stairs leading up to the front door of the program has chipped and peeling paint that is in need of repair.
Upon a review of IHCP's at the program, one child did not have an IHCP for a diagnosed medical condition. There must be an individualized health care plan for each diagnosed medical condition.
A review of three Staff Records at the program indicated the following: -Two Staff Records did not contain written documentation of observations conducted by a supervisor every two months 7.09(17)(b)(01).
Staff Supervision
The program does not have a current Building and Fire Inspection Certificate, as the last one expired on 10/14/2022. During the visit, the Director stated that the town does not currently have a Building Inspector and this is the reason for the delay.
