Children's records were missing complete enrollment packets and lead tests. Please obtain these forms from parents and add to record; confirm once completed.
Evacuation log was not complete. Please complete a fire drill monthly and document it; outline plan on how fire drills will be done monthly.
First aid kit was missing emergency cards/authorization for emergency treatment. Please utilize emergency contact form in enrollment packet and add them into the first aid kit; confirm once completed.
Educator has not taken sufficient trainings. Please ensure 10 hours have been completed from within the last year; upload certificates once trainings are completed.
When Licensor arrived for the visit, Educator was not on the same floor level as child and took an excessive amount of time coming downstairs. Educator's husband was on the same floor level as child. Additionally, the infant was sleeping in the with closed door. Please ensure an EEC caregiver always remains on the same floor level as children and confirm this will be done moving forward.
Infant was given a snack and did not have hands washed beforehand. Please ensure hands are always washed before eating; confirm this will be done moving forward.
Licensor walked through indoor space and the following hazards were observed: There were electrical outlets that were not covered; please add convers and ensure they remain on at all times. There was cat litter boxes accessible to children. Please make boxes and pet food are accessible to children and upload a picture once completed. Licensor walked through outdoor play space and the following hazards were observed: There were slides in the play space that exceeded 18 inches. Please ensure these slides are removed or install a fall zone. Please confirm once completed.
Smoke detector was tested and it was not in working order. Please replace batteries and confirm once completed.

Licensor walked through indoor space and observed the following hazards: There was an uncovered trash can in the bathroom. Please ensure a cover is added. Also, there was shampoo and condition accessible. Please make items inaccessible. Confirm once completed. In the kitchen, there was a barrel of plastic bags. Please make these bags inaccessible. Also, there was no lock under sink where there were cleaning products. Please add a lock. Confirm once completed. Kitchen- lock under sink , plastic bad

Stairways
Licensor completed a follow up visit to look at new fencing. The fence requires another post; please add this, confirm once complete and a follow up visit will be conducted. Outside of new second exit, there is a significant drop off accessible. Please install a safe, sturdy and 4 foot high fence that blocks access to the drop off. Please confirm once completed and a follow up visit will be conducted.

CPR and first aid is expired. Educator cannot provide care until first aid and CPR is renewed. Please obtain this as soon as possible and email copy to EEC.
First aid kit was incomplete. Please obtain a new kit and confirm once completed.
There are two exits on the main level, but they are side by side. There must be a separate exit on this main level. Please resolve this issue and confirm once completed. A follow up visit will be scheduled.
Chipping and Peeling Paint
Licensor went to the home to verify that peeling paint was corrected. Educator still had peeling paint on the backyard fence posting and on a fence towards the back of the yard. Please resolve all chipping and peeling paint on property.
Safe Sleep
Progress reports were not up to date. Please ensure all progress reports are updated and confirm once completed; also, include a plan on how progress reports will remain up to date moving forward. *For school age children, the progress report must be prepared at least annually, at the midpoint of the child’s program year. *For toddlers and preschoolers, the progress report must be prepared every six months. *For infants and children with identified special needs the progress report must be prepared every three months.
There was some chipping/peeling paint on the front door walkway and window sills of garage. Please ensure paint is removed; confirm once completed and a follow up visit will be scheduled.
There was an infant sleeping in an area that had loose items stored, which is considered a Safe Sleep hazard. Please ensure the Safe Sleep Pledge is signed, have parents sign Safe Sleep Parent Letter, and complete Safe Sleep training. Please provide documents/certificate to Licensor once completed. A follow up visit will also be completed within one week.
