Educator and her son in law were the only persons on the premises at the time of this visit. Educator's son in law does not have a current background record check and is not approved to reside in the home. Please complete the BRC process as soon as possible and ensure that no other household members or persons regularly on the premises are in the home without completed BRC's.
Space (indoor / outdoor hazards) - R Apr 2025
Licensor walked through indoor, licensed space and the following hazard was observed: In the master bedroom, there was exercise equipment accessible. Please make these items are inaccessible and upload a picture once completed. Licensor walked through outdoor space (fenced in backyard), and the following hazard was observed: The fence surrounding the play area was not secure. Please ensure all areas of the fence are sturdy, four feet high and have no spaces accessible for children. Please confirm once fencing is fixed and follow up visit will be scheduled.
This investigation inspection recorded no violations or advisories.
Evacuation Logs
Educator has two PQ #'s, one is expired and the other is "pending renewal". Please ensure PQ is active and renewed. Provide new certificate once renewed.
Annual Registration in Professional Qualifications Registry (PQR)
CPR Exp. Date: 12/22/2022 Educator completed a first aid course online during pandemic and this is no longer acceptable. Please renew first aid course and ensure it has an in-person component. Please submit the certificate once completed.

Licensor reviewed progress report and they were out of date. Please review all records and ensure all progress reports are updated.
There were children napping in a bedroom and the door was shut. Licensor discussed the napping policy with Educator. Please ensure all bedroom doors are ajar when children are napping.
Supervision - Napping Children
Educator has not kept evacuation log up to date. Please ensure fire drills are conducted monthly and documented in the log.
Licensor reviewed children's records and some files were missing enrollment packets and physicals. Please review all records and ensure the records are complete and up to date

Licensor walked through indoor space and the following hazards were observed: In one bedroom, there was an AC on the floor. Please install on the wall or remove from the space. In the same bedroom, there was a television that was not anchored. Please anchor to avoid a tipping injury. There was a power strip that was accessible. Please cover all outlets or make the strip inaccessible. Please confirm once all things are resolved.
