Non-prescription medication [redacted] and prescription medication [redacted] was expired for a child with [redacted] allergies in the upstairs classroom.
The backpack containing emergency medications was missing from the upstairs classroom, and emergency medication was not readily available for three children. An educator located the backpack and medications were reviewed by the licensor at the end of the visit.
This change of space visit inspection recorded no violations or advisories.

It was learned that at least one educator did not have documentation of current CPR present at the program on the day of the visit.
It was learned that the administrator and educators did not have current First Aid certification. The Director reported that First Aid and CPR training is scheduled on December 14, 2024.
During a review of children's records, at least one child was missing a physical and immunization records.
It was learned that the qualified administrator does not have 100% non-teaching administrative time as required for a capacity of 60 children.
It was observed that the pipes in the children's bathroom in the Ladybug Room are rusty and sharp and may cause an injury to children.
It was determined that the depth of the pea gravel does not meet the requirements in the fall zones and it measured 4 inches in three locations under and around the climbing structures.
It was observed that there are hazards in the outdoor space, including a sharp end of a nail is protruding from the barrier around the climbing structure, a green cover over a hole does not fit properly, an area around the air conditioning unit contains hazards, and there is a cracked window in the Butterfly Room.
It was observed that there is chipping paint in several areas on the outside of the facility and on the door leading into Butterfly Room.

Staff child ratios were not met in one classroom. One educator was alone with 11 preschool children.

During the review of children's records, one child's file was missing a current medication consent form.
It was observed that the window guards in the upstairs classroom pose a head entrapment hazard. From the bottom of the windows guards to the windowsill measure between 3 1/2-9 inches.

It was observed that part of the fence in the outdoor space was damaged and needs repair, and another part of the fence is temporary and needs replacing with a four foot high permanent fence.
During the visit, it was found that one child's IHCP was expired.

All educators must obtain within six months of employment, and must maintain thereafter current certification of training in basic first aid appropriate to the population served.
The licensee must maintain adequate first aid supplies, including, but not limited to: adhesive tape, band aids, gauze pads, gauze roller bandage, disposable non-latex gloves, instant cold pack, scissors, tweezers, thermometer, and CPR mouth guard.
IHCP and medication consent must be singed by parent.
