Chipping and Peeling Paint
There is a deck in the back of the house. Licensor observed chipping/peeling paint on the deck.
Educator tracks the children’s attendance in a notebook and documents the time of arrival and departure each day. However, at the renewal visit on Friday 4/4, Educator did not have attendance for any of the children for the entire week.
There are multiple children in the program with allergies and an epi pen. This information was not posted in the program.

Educator randomly reviewed 5 of the children's records. Of these, 3 of the progress reports were out of date.
Educator stated she has conducted a fire drill every month. However, she did not document any of the drills yet this year.
Licensor observed chipping and peeling paint on the railings and the flooring of the deck. There was also chipping paint on the door frame of the closet in the playroom.
Licensor inspected all of the licensed, indoor space during the renewal visit. The following hazards were observed: ** PLAYROOM **- peeling paint on door frame of closet ** DININGROOM **- no hazards ** KITCHEN **- no hazards ** LIVINGROOM **- storage unit not secured to the wall ** BATHROOM **- no hazards.
Exit Safety
First Aid Kit (FCC 2)
All of the required items were observed in the first aid kit. However, many of the children’s emergency contact forms were out of date.
There are 2 exits from the home, the front door and back door. Both exits were clearly marked. The front door was easily accessible, but the back door was not. Blocking the back door, were large plants and a large guinea pig cage. These items were blocking the exit.

Licensor inspected all of the licensed, indoor space during the monitoring visit, as well as the outdoor playspace. The following hazards were observed: ** PLAYROOM - no hazards ** DININGROOM - no hazards ** KITCHEN - knife block on the counter, cleaners in the cabinet under the sink ** LIVINGROOM - outlet not covered under the keyboard, cube storage is a tipping hazard and needs to be secured to the wall ** BATHROOM - hygiene products in the tub ** OUTSIDE - fire pit did not have a cover.

There are 2 exits from the licensed space, the front door and rear slider. Both of these exits were clearly marked and the front door was easily accessible. The rear slider had an animal cage blocking access to it.

Licensor reviewed 5 of the children's records. Of these, 4 were complete and up to date. The 5th record had no signature on pg. 6 of the enrollment packet for basic transport or topical ointment. Educator tracks the children's attendance in a planner notebook. The time of arrival and departure is written for some days, but not every day. Discussed that these times need to be documented every day for each child.
Licensor reviewed one of the children's records during the virtual visit. The record was complete and up to date, except for an overdue progress report.

Educator was not able to locate her current evacuation log during the virtual visit. She stated she would email it to Licensor, but Licensor never received it.
Outdoor Playspace - Hazards
Indoor Hazards
Licensor walked through all of the licensed space and observed the following hazards: **Playroom - no hazards **Dining Room - no hazards **Kitchen - drawer at the edge of the dining room needs to be secured as there are hazards inside. The cabinet under the sink has cleaners inside and the child proof lock was not engaged. Please ensure the cabinet's child lock is engaged during child care hours. The drawer to the left of the sink has sharp objects inside. Please remove the sharp items or lock the drawer. **Living Room** - outlets on power strips are not child proof. The fish food and fish tank cleaners are accessible to the children. Please make these items inaccessible. **Bathroom** - cleaners under the sink. Please remove items or child proof the cabinet. There were hygiene products in shower that need to be up at least 4 ft.
Licensor utilizes the side yard as her outdoor space. The only hazard that was observed is that the sandbox was not covered. Please ensure the sandbox is covered when not in use.
Licensor reviewed 5 of the children's records. Of these, 4 had a complete and up to date progress. The 5th record did not have a progress report. Please review all of the children's records and ensure that they all have a complete and up to date progress report.
Educator does not have a record for her assistant.
The children’s belongings, including coats, extra clothing, and bags, are stored on hooks, just inside the front door. They are all touching, which is a cross contamination concern. All of the children's personal items need to stored separately, so they are not touching each other.
