At the time of the visit, a snack in the kitchen was found to be expired, and a rotted orange was found in the children's refrigerator.
At the time of the visit, the following was found: 1) Chipping paint was found on the exit door of the Toddler One Classroom.
At the time of the visit, an Educator in the Pre-Toddler 2 Classroom did not follow diapering procedures. The following was observed: 1) The Educator did not keep at least one hand on the child at all times when the child was being changed on an elevated surface. 2)Two children and the Educator did not wash their hands after diapering.
Educators and children in the First Steps 2 Classroom did not wash their hands before eating and handling food.

At the time of the visit, the following was found: 1) Chipping paint was found in the First Steps 2, Pre-Toddler 2, and the Pre-K1 Classrooms. 2) The emergency door in the Infant 2 Classroom was leaking water. 3) The outlet in the First Steps 2 Classroom was broken. 4) The wall behind the Infant/Toddler Classroom door needs to be repaired.

During the program's safety inspection, the following was found: Chipping paint was found in multiple areas of the program.
In the Infant Classroom, One educator did not keep at least one hand on the child at all times when the child is being changed on an elevated surface.

During the health and safety review of the indoor spaces, the following was learned: 1) Chipping/ peeling paint was found behind the diaper area in the First Step Two Classroom. 2) The Dramatic Play Kitchen in Toddler 2 is not secured.
During the health and safety inspections in the First Steps Two Classroom at 11:08 a.m., the Licensor and Unit Supervisor noticed, through the windows, an educator of the Pre-K 2 Classroom was on the playground leaning against the play structure, looking down at the iPad, and not interacting or supervising the children. The Licensor and Unit Supervisor continued to observe the educator and then moved into the empty Get Set 2 Classroom, where they stood by the back door of the classroom and continued to observe the educator still leaning on the play structure, looking down on the iPad. At 11:22, the Licensor walked out onto the playground, and the educator looked up from the iPad and began to interact with the children. The Licensor checked the attendance for the Pre K-2 Classroom while the Unit Supervisor held the door for the Licensor. Licensor entered the building at 11:23.

At the time of the visit, the following was observed while reviewing all indoor licensed spaces: In all Toddler and Preschool classrooms, multiple shelves and dramatic play equipment needed to be secured.

Nine staff records were randomly selected for review, and the following was learned: 1) One Staff needed evidence of 2 doses of [REDACTED] and a current physical as required by regulation. 2) One Staff did not have a current EEC PQ Registry or a completion certificate of the EEC Essentials Training as required. 3) One staff did not have evidence of a resume, date of interview, two references, and EEC PQ Registry as required by regulation.
Five children's records were randomly selected for review, and the following was learned: One child was missing a current physical and evidence of recent Lead screening. 2) A child who was enrolled in the preschool room as a developmental placement was missing evidence of the required monthly progress documentation.
A box of Cheerios was found open and not sealed properly.
Random review of 4 staff files found the following: 3 files did not contain evidence of immunity to [REDACTED]and 3 files did not contain evidence of a current physical.
Classrooms did not have their emergency medications posted. One educator stated it was in their binder, but someone had taken the binder out of the classroom.
An educator was observed changing an infants diaper and then placing them on the floor without washing their hands.
Random review of 6 children's files found the following: 2 files had no required medical information (current physical, current lead and immunizations) and 1 file did not contain a current lead screening.

An Educator in the First Step 2 Classroom did not wash her hands between changing children's diapers.
During a safety check of the preschool playground, the artificial turf was found to pose a potential tripping hazard to children.
During Health and Safety inspections in multiple classrooms throughout the program, the written emergency/evacuation procedures are posted next to each exit.
Nine staff records were randomly selected for review, and the following was learned: 1) Three Staff did not have evidence of [REDACTED]2) Two Staff did not have evidence of a current physical as required by regulation. 3) One Staff did not have a current EEC PQ Registry. 4) Two Staff did not have evidence of a completion certificate of the EEC Essentials Training as required. 5) Two Staff did not have evidence of two references, and Four Staff only had evidence of one reference.

During an activity in the Infant/Toddler classroom, an educator was not nurturing and responsive to the children's needs. The educator removed materials from the table before the children completed the activity.
At the time of the visit, the following was observed while reviewing all indoor licensed spaces: In all Toddler and Preschool classrooms, multiple shelves and dramatic play equipment needed to be secured.
At the time of the visit, the following was observed while reviewing all indoor licensed spaces: 1)The door handles were broken on the closet gym doors and would not stay shut, creating a potential risk to children. 2) The alarm to the emergency door in the Infant/Toddler classroom was beeping and needed to be replaced. 3) Infant one and Preschool one classroom brooms were not appropriately stored as required by regulations. 4) A child-proof magnet was left on the diaper changing door within reach of children.
At the time of the visit, both preschool classrooms' second emergency exits did not post the appropriate emergency exits posting.
At the time of the visit, the following was observed while reviewing the kitchen: 1) An open container of animal crackers was not dated. 2) A cheerios box was dated but was not stored in a clean covered container.
It was observed in the Infant/toddler classroom that the educator did not keep one hand on the child at all times while completing a diaper change.
During the visit, it was observed that the walkways on the playground were not shoveled, causing a potential risk for children in an emergency.
