A random review of children's files showed that not all required yearly updates had been completed. Some files were missing medical records, lead test results, progress reports, Individual Health Care Plans (IHCPs), and medication consent forms. Additionally, some IHCPs and medication consent forms were missing the required signatures.
There was no documented evidence of regular staff meetings or ongoing professional development.
Random review of children files revealed a child's file did not have current medical records. The last record on file was from 2022.
