Four records were reviewed at this visit. The records were missing an annual update form and one record was missing proof of current medical form.
The staff record was incomplete. The record was missing the medical form, the assistant's attendance log and proof of training hours. The assistant's PQR must be active and trainings need to be listed.
The educator must provide proof that she has completed the required training hours.

The water temperature was exceeding 120 degrees F. Plastic bags were accessible on the first floor of the home. Plastic bags must be kept inaccessible. The playground structure does not meet the required fall zones. All sides of the structure must cover at least six feet on all of the sides.

The program had two separate exits. The exit that leads to the back yard needs to be posted.
The first aid kit was missing a gauze roller and the parent emergency contact information.
The sensory table with beans that was located in the playroom needs to be removed to an area that is not used with infants and toddlers.

Outdoor Playspace - Hazards
Two of the children's records were reviewed. One record was missing an updated physical and lead exam. One record was missing a current lead exam. Both of the records were missing an annual update form.
The staff record did not include the required documents. The program works with a regular assistant.

The water temperature was reading at 146 degrees F. The water temperature must not exceed 120 degrees F to prevent scalding. Please provide picture or video showing new reading.
The outdoor playspace is not protected with a permanent fence. The permanent fence was removed due to some construction. The home is located on a busy street and requires a permanent fence. Please indicate an estimated date of when the permanent fence will be installed and replaced.
The assistant's letter of approval was not posted.
