1) Some medications were expired at the time of the visit. 2) A staff member did not wash hands before and after administering medication to a child.

During the visit there were more than 12 children in the snack room.
Garbage kept in lined and covered containers, emptied daily
At the time of the visit the garbage cans were not covered.
Care provided only in EEC approved space; 35 square feet of activity per child.
At the time of the visit there were not enough materials for the children in the classroom and in the gym, especially for children who did not want to participate in the gym activities.
A written progress report must be done periodically
Equipment is clean, safe, secured, and poses no hazard.
1) Loose cables/cords were observed in one classroom. 2) A window leading to a roof was open.
Staff effectively communicate with each child
Teacher communications were inappropriate. The following conversations were heard: 1. A staff member said to a child "You're so miserable." 2. Yelling a countdown to the children to get them moving. 3. Singling out one child telling them "we're waiting on you" 4. Staff member telling the group that they are waiting for silence. 5. Staff threatening to take away gym time and telling the students they are wasting their own gym time. 6. A staff member told a child to go to the end of the line because they cleared their throat.
A sample review of children's records showed that Individual Health Care Plans were incomplete.

Staff monitor environment for hazards-Hazardous items not accessible to children
One staff record was missing a physical and first aid.
A school-age administrator needs to be assigned to this site, the administrator that came to the program on the day of the visit is the full time administrator at another full time program.
During a sample review of children's files progress reports were not in their files.
Staff were not nurturing, supportive or responsive to the children. Many of the interactions between the staff and the children did not support the development of self-esteem, independence and self-regulation.
Transitions were not planned and very chaotic. Children were climbing on the bleachers.
At the time of the visit the curriculum was not well-balanced and it did not support the social and emotional development of the children.
Children were waiting for long amounts of time, and teachers were talking inappropriately to children.
There were two fans accessible to the children in the classroom and one in the gym that had a cord handing within reach of the children.
Classroom was dirty (counters, microwave, stove and fridge). There was a bottle of bleach in an unlocked cabinet. There was a space between the wall and shelf and children were playing there.
At the time of the visit the playground had a metal pole on the ground.
The allergy list was posted on the inside of a cabinet door that was wide open during the visit, not protecting privacy.
The documentation showed that there were several months without fire drills.
At the time of the visit there was food that was open and cereal that was expired.
Teachers backpacks were within reach of the children.
