Some of the records were incomplete. *One record was missing current physical, immunizations and lead exam. *One record was missing the child's history information and some of the pages were not complete. *One record was missing the immunizations record and the annual update form. *A couple of the records were missing the educator's and child's name on the required fields.

The following outdoor hazards were observed: *A plastic bag was accessible on the back deck. *The outdoor space had a large ladder that was accessible. * A weed machine was accessible *A pile of large boxes, plastic boards and other materials were accessible. *A grill was placed on the back deck that was not covered. Indoor hazards: *The water temperature was set to 126 degrees F. The water temperature needs to be adjusted to not exceed 120 degrees F.
This investigation inspection recorded no violations or advisories.
Some of the records were missing parent's signatures on the required fields on pages 3 and 6 of the enrollment records. Some of the records were missing the educator's name on the required fields. One record was missing the lead exam and proof of vaccinations.

The staff record did not include the assistant orientation form, and the CPR and FA certificate provided stated online only with a 90 day to perform the skills.
The ladder on the deck needs to be removed from the play area.
The outdoor play area needs to be clean. The large rocks on the side of the yard need to be removed to prevent injuries. The outdoor space did not have outdoor play materials.

The home had a front and a back exit. The exits need to be posted.
A gate will need to be placed between the kitchen and dining room. The bedroom was missing a door. If the door to the bedroom is not replaced a gate must be placed. Hazards in the bathroom: a door needs to be installed, a door frame at the bottom of the floor needs to be replaced, soap and disposable towels need to be accessible for hand washing, the space heater in the the bathroom was missing a cover, the bathroom did not have a working faucet. The kitchen floor needs to be cleaned. It had dust from repairs that were done to the bathroom.

Refrigerator Temperature
No carbon monoxide device observed. This is a housing complex. Provider will check with management and report that one was installed. Pictures will need to be submitted via the report or emailed to the licensor.

CPR/First aid not verified. Educator reported class taken on the 25th. Educator will send a copy via email to the licensor or attached to this report.
No thermometer present during this visit. Thermometer needed to test temperature. Must be between 32F and 40F degrees.
The water temperature was exceeding 120 degrees F. The temperature was set to 147 degrees F.
The home has a deck that will be used to exit to the backyard. The gaps between the spindles on the deck need to protected on both sides of the deck. The gaps were measuring higher than 3 1/2 inches apart which can cause a head entrapment. The deck needs to be cleaned and clear of dust.
