At the time of the visit the school age program were lacking activities that were developmentally appropriate.
At the time of the visit teachers were not following diapering procedures.

Toxic Substances
On the day of the visit staff reported that there were four infants enrolled in the infant toddler room.
In several of the classrooms there are not enough materials for the children. In preschool room 2 there were not enough materials in the kitchen area. The school age rooms did not have materials for the children. The only accessible materials were books and crayons. There were empty shelves in the classrooms. The infant/toddler room needs more materials for the toddlers in the classroom.
The following were observed during the visit: In the Pre-K room there was a lot of clutter on top of the cubbies and throughout the room. There were rusted shelf brackets within the access of the children. In one of the bathrooms there was a plunger accessible to the children. In preschool 2 there were 2 plastic bins that were broken (one with sharp edges) in the cubbies within the reach of the children. The closet floor in the school age room was very dirty. Toothbrushes in the school age room were stored in an unsanitary way. The toddler bathroom area was being used as storage and was extremely cluttered and did not allow access to the sinks.

There was not enough materials available to the children in the school age classrooms.
During the review of children's records there was missing information, including: dates of progress reports, and developmental history information.
Laundry room was not locked and accessible to the children.
The space for the school age children was not appropriate. The gross motor room was not being used appropriately.

There was missing information in the staff files during the file review. Missing items included: no physical, no first aid, missing qualifications, no identification, incomplete BRC information, no staff information sheets for school age staff showing their current position.
Attendance was not accurate in the Pre-K classroom.
The teachers in the school age program were not providing adequate supervision of the children.
At the time of the visit the school age program did not provide for the development, interests and temperaments of individual children.
In one of the preschool classrooms the cover was missing from the trash can.
At the time of the visit children were not washing hands after using the bathroom (licensors had to ask them). It was also observed that a staff member kept on dirty gloves while holding a child's hand before she washed her own.
At the time of the visit in the toddler 3 classroom medication was locked in a closet making inaccessible to staff.
There was missing information on the Individual Health Care plans. Specifically who was trained on the medical condition. There was also a missing signature on on of the plans.
During the visit it was observed that a teacher did not wash her hands after cleaning a table.
At the time of the visit the toothbrushes were touching.
One of the classrooms had chipping paint at the time of the visit.

During the visit it was observed that a child was trying to sit in a chair where another child was sitting and the teacher picked up the child and moving instead of helping them resolve the situation.
Paint is peeling in some of the classrooms, around door frames and doors.
It was observed that the evacuation procedures were not posted at every exit.
It was observed that bottles of hand sanitizer were within the reach of the children.
A sample record review showed that a child with a chronic medical condition did not have a completed Individual Health Care Plan.
At the time of the visit the medication was not stored appropriately and not accessible in the event of an emergency. Medication for one of the children was missing from the first aid bag in the preschool room.
Through interviews it was learned that bottles are not being sanitized after each use. It was also observed that two bottles in the refrigerator were not labeled.
Could not access rescue medication due to closet being lock.
During the visit it was observed that nap mats were stored with sheets on them touching each other or with pillow cases that were touching each other.
