Three of the children's records were reviewed at this visit. The records were missing the following: *One record was missing parent signature on page 5 of the enrollment record and record was missing current physical, immunizations and lead exam. *One record was missing medical form, or the school age section needs to be signed by the parent. *One record was missing permission for walks and parks.
The playpen was missing a fitted sheet.

The step ladder located on the hallway that leads to the bathroom needs to be removed and placed in an area that is not accessible to the children in care. The outdoor space was properly barricaded.
The program needs to add trainings to the PQR.
The family room was cluttered with clothes. The area needs to be clear of clothes.
The children's records were missing the following: 1.) School age section regarding physical forms needs to be completed for the school age children by their parents. 2.) One record was missing proof of lead exam. 2.) Permissions for the indirect supervision needs to be updated. 3.) Page 5 of one of the records was missing parent signature and topical ointments need to be listed.

This follow-up visit inspection recorded no violations or advisories.
Plastic bags were located in the living room. Plastic bags must be kept out of the reach of the children. The first aid kit was missing family emergency contact information.
The program was not following proper safe sleep practices. The nine month old infant was sleeping on a couch with a blanket on their side.

The playpen used for the toddler in care did not have a fitted sheet. The program had cots for the preschoolers and a crib for the infant in care.

The home had peeling paint on some of the window boards. The home needs to be free of peeling and chipping paint.
The grill needs to be protected with a cover. The outdoor play area is fenced in.
