Supervision of Children
On 2/10/26, a preschool-aged child was found unsupervised by a parent at the front entrance of the building. It was identified that the side fence gate was left open while the child was outside with their group.

A review of three staff records indicated that one did not have a signed staff information form and two did not have evidence of EEC Essentials training completed.
During the visit, it was observed in the DAFFODILS classroom that the staff to child ratio was not maintained when an Educator took children to the bathroom, leaving the other Educator alone with 11 children.
A current staff schedule was not posted.
A review of six IHCPs indicated that one did not have a completed IHCP, and one did not have a completed parental medication consent.

Evacuation drill log, all groups
It was observed that in the SUNFLOWERS classroom, one of the bookshelves was not secured and tippable.
Equipment is clean, safe, secured, and poses no hazard.
A review of the evacuation drill log indicated that a drill was not documented for the months of May and September 2023.
A review of six staff records indicated that three did not have evidence of a current physical examination.
A review of the staff meeting log indicated that monthly meetings were not consistently at least two hours long.

The following was observed and accessible to children: The carpeted wall along the ramp had a ripped seam. In the BOY'S BATHROOM, the floor mat was worn and ripped, and a part of the vinyl baseboard was loose. In the hallway, the floor mat was worn and ripped, and there was a cracked floor tile. In the DAFFODILS classroom, the carpeted floor had two ripped seams.
A review of seven children's Individual Health Care Plans (IHCP) indicated that two did not indicate which staff were designated to administer medication.
A review of medications indicated that one prescribed medication was not stored with its original container.
It was observed that in the DAFFODILS classroom, there was chipped paint.
