A Medication Consent Form did not include the date the Form was signed for a child prescribed an (REDACTED).

Upon review of three Children's records, one record had outdated signatures on all EEC required forms. The signature date was 2/4/2020 and this needs to be updated annually.
A review of three children's files indicated: -Three files did not have a current Individual Health Care Plan (IHCP) for children with (REDACTED) . -Three files did not have Medication Consent Forms for (REDACTED). *Individual Health Care Plans must include a description of the (REDACTED), its symptoms, any medical treatment that may be necessary while the child is in care, the potential side effects of that treatment, and the potential consequences to the child's health if the treatment is not administered.

A review of three staff files indicated: -One file did not have current First Aid training.
A review of three staff files indicated: -One file did not have evidence of a completed Background Record Check (BRC) Consent form.
The Program did not have a Staff Schedule posted during the time of the visit.
The following were observed and accessible to children throughout the Program: -Shelving units with baskets were unsecured. -Trash containers were uncovered and did not have lids. -Behind the door of classrooms were brooms and dustpans. -In the Library area, a projector had a wire longer than six inches. -In the Preschool 4 classroom, the window trim was exposed and in process of being repaired. -In the hallway, there were standing air conditioning units that were unsecured.
The Building Inspection Certificate expired on 8/1/2019.
A review of medication stored at the Program indicated: -Required medication for three children with a (REDACTED) were not available on site. Therefore, three children in the program did not have access to (REDACTED) in the event of a possible emergency.
