Staff records were missing evidence of bi- monthly observations and one staff was missing evidence of MMR immunizations.
All children’s records were missing a transportation plan, indicating how children arrive and depart the program.
The emergency evacuation drill log was missing the month of February.

Two toddler rooms attendance did not indicate the departure times of children on several days.
1. At the time of visit the toddler 3 room had seven children listed on the attendance but six children were present. One child was not signed out that day. 2. A child from toddler 4 room was napping in toddler 3 but was not signed in on the attendance. 3. Two toddler rooms had incomplete attendance for the months of November and December. 4. Toddler room 2 was not completed for the month of December. 5. The attendance did not indicate the arrival and departure times of children.

The program operated out of their licensed capacity and provided care for more than the maximum number of children indicated on the license. for the toddler groupings. 2. The program current building inspection only included 9 toddlers and the program had over 9 toddlers enrolled.

1. One child was missing a developmental history form, evidence of an annual physical examination and immunizations. 2. Two children files were missing developmental history and transportation forms.

One emergency medication was stored in the nurses office on the first floor and was not immediately available for use as needed,. 2. One emergency medication was not in the containers in which they were originally dispensed and with their original labels affixed.
