However, a review of her assistant's files indicated that her Certified assistant [REDACTED] whose certificate was renewed on 06/17/2024 completed only 11 hours of professional development training during her previous licensing cycle; Certified Assistant [REDACTED] has not completed any hours of professional development training since her certificate was issued on 09/08/2022. At the time of the visit, the licensor observed one of the childcare child punching and pushing another childcare child three separate times and the assistant did not do anything about it. The licensor had to intervene by stopping the child that was punching and pushing the other child. Then, she addressed this matter with the assistant [REDACTED] who then proceeded to talking with the child that was pushing and punching the other child about appropriate and inappropriate interactions with their peers.
kitchen cabinets and drawer contains plastic bags, detergents and other hazards within children's reach and the locks are in disrepair.
A review of the children's enrollment form and medical record indicated that 3 children enrolled in the childcare program have a medical condition. The educator does not have this information posted anywhere in the program, does not have the children's Individual Health Care Plan in place or the medication at the program.

There is a closet in the bathroom that is open and accessible to children and has hazardous items in it such as long electrical cords and tipping hazards.
2 children were observed leaving the Playroom, without supervision and moving to the hallway and to the Kitchen without being supervised while the educators were unaware of the children's activities for several minutes.
The green grass-like rubber rug in the outdoor space has mold on it, which is hazardous and accessible to children. There is a small individual trampoline in the outdoor space that is accessible to children.

The children were observed doing one activity together, while others were wandering around the Large Playroom, pushing high chairs and climbing on equipment. There was no evidence of structure or routine in the program.
There are not ample materials for the children enrolled. 4 children were given one coloring book to color in, while 2 children were wandering around the Large Playroom, looking for something to play with.
1 mat is ripped in a couple of different places and is not in good repair.

An 11 month infant was observed sleeping on a mat on the floor with a blanket wrapped around them.
Kitchen: the cabinet and drawers contains knives, plastic bags, medicines and other hazards in them and they are not child proofed.

Some children’s enrollment forms are incomplete and missing parent signatures on permission to administer CPR/FA in an emergency and other parts are left bank. Some children’s medical records are due for update.

The educator is not completing the children’s progress report. Educator stated all the children are newly enrolled, however, a review of the children’s enrollment form indicates that most of the children have been enrolled in the program for over 6 months.
It was observed that the assistants barely talked with the children or engage in social conversation with them. The assistant mostly interacted with the children when they had to redirect their behaviors.
When licensor arrived, licensor observed all the children sitting in a row and watching music on the assistant’s telephone in the playroom. The assistant shut the telephone immediately after seeing the licensor. Afterwards, the children started running and walking from one end of the room to the other without any structure. The assistant that was in the kitchen came to the playroom and organized the children around the table and gave them coloring paper and crayons to work with.
