At the time of the visit, Student B's prescribed medication was not in the original box and was missing the prescription label. All prescription medications must be in the containers in which they were originally dispensed and with their original labels affixed. Over-the-counter medications must be in the original manufacturer's packaging.
At the time of the visit, in a review of the current staff record checklist. It was identified that Educator K was hired on 5/28/2024 and did not have documented First Aid Training. The licensee must document that each educator is certified in first aid within six months of employment.

Current consent for administered medications
At the time of the visit it was observed that an educator within the Toddler Classroom left ratio to get the students mats/cots which were stored outside of the classroom. This left one educator in ratio alone with eight students. Educators must not engage in any other activities or tasks that could unnecessarily divert their attention from supervising the children.

At the time of the visit in a review two IHCPs the documentation was not fully complete. Both IHCPs were missing documentation of the educators trained on the health condition. One IHCP was missing documentation of the side effects of treatment. The licensee must maintain as part of a child's record, an individual health care plan for each child with a chronic medical condition, which has been diagnosed by a licensed health care practitioner. The plan must describe the chronic condition, its symptoms, any medical treatment that may be necessary while the child is in care, the potential side effects of that treatment, and the potential consequences to the child's health if the treatment is not administered.

At the time of the visit, in a review of three staff files the information was not fully complete. One educator was missing documentation of an annual evaluation. Another educator had an expired annual evaluation. The Program must maintain a personnel record for each staff member, including immunization records
At the time of the visit, in a review of three students files, two students had annual off site consent that was expired. Children's records must have all required applicable documentation according to the Children's Record Checklist.
At the time of the visit, in a review of two IHCPs and the corresponding prescribed medication, one student had prescribed medication that was stored outside of the original box and was without a prescription label. All prescription medications must be in the containers in which they were originally dispensed and with their original labels affixed. At the time of the visit, in a review of two IHCPs and the corresponding prescribed medication, the medication was not readily available for the Infant and Toddler Classrooms. The medication was stored outside of the classrooms within a wall mounted medication box. Two student were prescribed antihistamine and did not have their medication on site. Emergency or life-saving medications, for any children for whom they have been prescribed must be readily available.
At the time of the visit, in a review of two IHCPs and the corresponding documentation, the documentation was not fully complete. Both students were prescribed two medications each; one student was missing a medication consent for both of their prescribed medications. The other student was missing medication consent for one of their prescribed medications. One medication consent was missing documentation of the dates for the medication to be provided, the times for the mediation to be provided and the directions for storage. All medications must be accompanied by written parental consent and healthcare practitioner authorization when required, including proper administration of medication.
Physical Facility Requirements
At the time of the visit, the play area did not have a structure that would provide an adequate amount of shade. The outdoor play area must provide for both direct sunlight and shade.

Upon review of the injury log, the corresponding incident report could not be located within the child's records.
A student within the toddler classroom was eating lunch in a chair that was too large for them. Staff were observed assisting the child into the seat, as the child could not get into the seat themselves. (7.07) (13)

A review of staff records indicated the following: One did not have a signature on their staff information form, two did not have evidence of EEC Essentials training completed, two did not have evidence of current EEC PQ registration, one did not have evidence of a job application or resume, and two did not have evidence of two reference checks.
A plan for diapering and toileting was not posted in the children's bathroom.

It was observed that the diaper changing surface was not cleaned and disinfected between each use.
A review of one individual health care plan indicated that it was expired along with the parental medication consent form.
