Emergency Evacuation
At the time of the visit, in a review of four educators files, the documentation was not fully complete. Educator A had an expired PQR. Educator B had an expired physical. The program must maintain a personnel record for each staff member as outlined by the staff record checklist.

At the time of the visit, in a review of four classrooms fire drill logs, Classroom 12 had incomplete documentation. Classroom 12 was missing documentation of May-August 2025 fire drills. The educator in the room stated the drills were completed, but they did not log them. The educator must hold practice evacuation drills with all groups of children and all educators from each floor level of the approved space at least monthly. Drills must be held during different times of the program day, and must use alternative exits. The educator must document the date, time, exit route used, number of children evacuated and effectiveness of each drill. At the time of the visit the Licensee stated they would find out when the drills were completed based on documented conversations held with the classroom educator. After the visit, the Licensee emailed the Fire Drill Log completed for Room 12.

At the time of the visit, in a review of five IHCP's and the corresponding medication consent documentation, one medication consent was not fully complete. It was missing documentation of the mild reaction symptoms to look for prior to administering the prescribed (REDACTED).

At the time of the visit, the following was observed: two shelves within Classroom Six and one shelf within classroom twelve were unsecure and tippable. A cord to the window air conditioner was accessible to children and a potential hazard. The paint on the bathroom stall doors and the heaters within the bathrooms were peeling. All indoor and outdoor materials must be secure and non-tippable. All electrical cords must be arranged so they are not a hazard to children. The licensee must maintain the interior and exterior of the program in good repair, free of chipping, flaking, or peeling paint or broken plaster

Individual Health Care Plan (IHCP) - R Apr 2025
It was observed in the first Children's Bathroom, hanging off of a low shelf, there were plastic bags accessible to children; potential cause for a choking hazard.

A review of stored medication indicated that one prescribed medication had expired as of (REDACTED).
A review of ten Individual Health Care Plans (IHCPs) indicated the following: Two did not have a doctor's signature. One was not fully completed to indicate which staff are trained to administer and who were they trained by.
It was observed that the emergency medications were stored in a locked storage room. The medications were not readily available in the case of a potential emergency.
