Four children's records reviewed had not been updated annually. There was no documentation of current physical exams in the records.
The educator had not completed written progress reports and provided them to parents as required(every 3 months for infants, 6 months for toddlers and preschool.)

Records were reviewed for the 9 children present; 6 needed Annual Update forms, 2 needed signatures for Acknowledgment of the Parent Handbook, and 1 needed a signature for the General Permission section.
The Educator had the children wipe their hands with baby wipes before snack. Discussed that children must wash their hands with liquid soap and running water. Also discussed that the Educator must was her hands and the children’s hands with liquid soap and running water after diaper changes.

The emergency preparedness plan was for the Educator’s former address. Discussed the plan needs to be updated to the Educator’s current location.

The progress reports were not current for the enrolled children.
The Educator did not have records for the assistant. Discussed that she needs to have a medical form, attendance, and proof of the EEC required trainings. The certification and CPR/First Aid cards were posted for the assistant.
The Educator had a cloth cover over the changing mat. Discussed that the cover cannot be used because it cannot be disinfected. The Educator removed the cover. The Educator later changed a diaper, but did not disinfect the diapering surface. Discussed that the surface must be disinfected after each diaper change. She did use a disposable surface on top of the vinyl mat, as required by EEC regulations.
