In the small front classroom, a medical condition list indicated that a child had a medical condition. When asked, a newer staff did not know if any children in the room had medical conditions. A review of the file indicated that the child had documentation from 2023 but nothing recent.

A review of staff records indicated that: -Evaluations were not always dated. -Bi-monthly observations were not signed and dated by who observed and by the staff who was observed (to indicate they were reviewed)
Three lunchboxes were observed beside the handwashing sink. In addition, a mesh wire basket containing paper plates and plastic spoons was sitting at the back of the sink. Facilities used for hand washing after diapering or toileting must be separate from facilities and areas used for food preparation and food service.
Of the 5 cups that were observed on the playground table, 2 were not labeled. Personal items intended for individual use by children, including but not limited to bottles, pacifiers, toothbrushes and sleeping materials, must be labeled with the name of the child for whom they are intended.
-Evacuation procedures were not posted at the connecting door in the small front room. -There was an open trash bag of diapers laying on the floor in one room, near the diapering area. -One cube cubby outside the Director's office, was tippable.
Two slides did not have a 6-foot use zone.

The following were observed during the visit: -A cord over six inches was accessible to children. -There was an airve3nt that was dusty in one classroom.
