Outdoor Space - R Apr 2025
Evacuation procedures were not posted next to each exit.
The following were observed: -Cords over 6 inches were accessible in the SA room. -In the one preschool room, two shelves were tippable and an apple corer was within reach of children. There were stained ceiling tiles.
On the I/T playground the following were observed: a metal band that had a sharp edge around the downspout. In addition, one hole in the ground surfacing could be a trip hazard and the door trim was broken. On the PS playground, there were protruding bolts.
The large composite structure was missing a step and was accessible to children. In addition, the swings were raised to the point that children could not touch the ground independently.

The following were observed: -A cord over six inches was accessible to children. -More than one shelf was tippable. -a plastic (grocery store style) bag was hanging on a door, accessible to children.
There was chipped/ chipping paint throughout the program (walls) and on the large composite structure outside
This investigation inspection recorded no violations or advisories.
During the visit, there were cords behind one crib that an infant was sleeping in. The cords could be easily accessible to the child, through the slats. In two other rooms, cords over six inches were not secured and were accessible to children.

A trash can containing food refuse was uncovered.

The attendance record in one room indicated that 7 children were present however 8 were in attendance.

At 11:00 AM, a container of hummus was sitting on a shelf. This had been part of the morning snack.
Outdoors there were two sections of the outdoor ground surfacing that were loose, creating a potential trip hazard. There were several areas on the composite structure that had broken wood and peeling paint.
This renewal - monitoring visit inspection recorded no violations or advisories.
The following were observed outdoors: -There was a cracked plastic container lid. The crack could cause a potential pinch point. -The door to the shed was not attached and was laying on the ground, creating a trip hazard.
Evacuation procedures were not posted at the exit to the outdoors in the infant room. Evacuation procedures in the infant room should identify how infants are evacuated.
An observation of two children's water bottles indicated that neither were marked with the child's name or initials.
The changing surface in the infant room had two (approximately) 1.5 inch cuts through the changing surface.
INDOORS: There were small amounts of chipping paint throughout the program. OUTDOORS: The program's large composite structure had a significant amount of chipping paint. In one area the wood was also eroded. There was a small amount of chipped paint on the fence.
