Six child records were reviewed during the visit and found two records were missing current physicals.
Two staff records were reviewed during the visit and found one staff record did not have evidence of immunity to MMR.
One child's IHCP was not complete. It was missing the names of educators trained to administer medication.

In ACORNS, one hole is visible on the left side wall in reach of children.

Two children's IHCPs were not complete. One IHCP was missing the symptoms, side effects of the treatment, and consequences. A second IHCP was missing symptoms and treatment.

The attendance in PINECONES was inaccurate. Nine children were present but only eight children were checked in. This was fixed during the visit.

Four child records were reviewed and found: -One child's record did not have a medication consent form for (REDACTED).
The Building and Fire Inspection was not current (expired 5/1/2023).
In the PINECONES classroom, the two far corners of the room had holes in the walls from previously used furniture anchors. The back corner had two holes on the wall with plaster visible and the the front corner had four holes on the opposite walls with plaster visible.
This investigation inspection recorded no violations or advisories.

The following was observed to be accessible to children: -In ACORN, two low bookshelves were not secured and are tippable. -In the ART ROOM a tall bookshelf on the right side was was not secured and is tippable. -In CHESTNUT, two wall vents near the floor are rusting.

After reviewing two Staff Records the following items were observed to be missing: -Both Staff Record's did not have current PQ Registry Certificates. One expired 12/28/2022 and the other expired 10/8/2022.
After reviewing three Child Records the following items were observed to be missing: -All three children's' Individual Health Care Plans were missing the names of staff trained to administer their medications and whom they were trained by. -Two of the children do not have current physicals. Their last physicals were on (REDACTED)
The wooden baseboards, chair rails, doorframes, and doors throughout the program were observed to have chipping paint.
The evacuation procedures were observed to be missing at the second egress in the ACORN classroom.
This enhanced monitoring visit inspection recorded no violations or advisories.
One carpet had a turned up corner, creating a trip hazard. One white cubby shelf did not have a protective corner cover. Two wires are protruding from a wall in hallway.
