During the visit, it was observed that two physicals were expired, one physical did not include the completion date, and one enrollment packet was missing the date of admission. The licensor encouraged the educator to review all children’s records to ensure that each file is complete and up to date.

During the visit, it was observed that the kitchen play set was not secured to the wall.
The emergency preparedness plan in the Parent Handbook did not list an evacuation location outside of the neighborhood. The educator reports the location is the nearby school. Discussed that it needs to be documented in the plan.
There is no evidence of ongoing professional development training for the regular assistant. The assistant has recently completed the EEC Essentials 2.0 trainings for 12 hours, but has not completed any other trainings.

Records were reviewed for the 12 enrolled children. Ten of the children needed Annual Update forms. The General Permission sections of several enrollment packets were not accurately completed and/or signed. Three of the children did not have proof of current physicals, immunizations, and lead screenings.
The license for the assistant was not posted.
The progress reports for the children were current, however the educator has only been completing them once a year for all children.
There was one child with a life-saving medication and allergies. This medication had expired.
The refrigerator temperature exceeded 40 degrees. Discussed the temperature must be between 32 and 40 degrees. Please send proof of temperature.

There was not the required 3 ft of space between mats for napping.
An outlet in the living room was uncovered. An outlet near the table in the playroom was observed to be uncovered.
