Current consent for administered medications
At the time of the visit three children with emergency medical conditions were reviewed and the following was found: one child was missing a medication consent form for one of their prescribed meds and were missing parent signatures on medical documentation.

During a visit conducted on 3/15/24 a bottle warmer was being used in an infant classroom. Items that become hot enough to burn a child must be kept out of reach. See bottle warming policy.
During a visit on 3/15/24 an infant was drinking a bottle while laying on a boppy pillow with no staff around. Infants are to be held when having bottles.
The following was observed and accessible to children: PRESCHOOL -Plastic bags were used to store children's sleeping materials. TODDLER 3 -Plastic bags were used to store children's sleeping materials. INFANT 2 -Three plastic bags were hanging in a storage area accessible to children.

The following was observed and accessible to children: PRESCHOOL -A plastic bag full of plastic bags was hanging on the door handle of the bathroom. PRE-K -A plastic play kitchen structure was not secure.
A review of the emergency evacuation log indicated that a practice drill was not documented in April 2022. The Program Director stated that a drill was completed. The educator must hold practice evacuation drills with all groups of children and all educators from each floor level of the approved space at least monthly.
An (REDACTED) stored at the program was not in it's original prescription box and did not include a prescription label as required. All prescription medications must be in the containers in which they were originally dispensed and with their original labels affixed.
