A review of 20 children’s records showed that: 4 children who are transported by vehicle do not have permission to be transported. 3 general permissions are signed but blank, 1 child needs a medical and 2 children need proof of a lead test

The (REDACTED) old infant was sleeping in a pack and play with an extra mattress on top. All of the pack and play’s have these mattresses. This is the 2nd citation of safe sleep, the first one was in 2017

The first aid kit did not contain the emergency contact cards for the children or the CPR mouth guard. The first aid kit was not taken outside today
Handwashing after diapering was done in the kitchen sink, it cannot be done in a sink used for meals

The licensor observed an assistant changing a child’s diaper. The assistant did not wash the infants hands after diapering The children did not wash their hands before eating snack outside

The educator installed a new swingset. There is not the required 6ft. Clearance around the swingset where nothing else can come in contact with it
A review of the attendance records show multiple days, at least 7 in the last 2 months were there were 11 or 12 children present
