At the time of the visit, BRC suitability letters had not been placed in staff files.
At the time of the visit three staff files were reviewed and the following was found: one file needed an updated physical, and two files did not have orientation checklists.
A review of four children's records indicated that one child did not have a complete Individual Health Care Plan. The name of the child was shared with the Program Director during the exit interview.

A shop-vac was observed in the pre-Kindergarten classroom accessible to children.
The following was observed and accessible to children: INFANT CLASSROOM -On the handwashing sink, there was an air freshener can and two disinfectant cleaning bottles. These items were accessible to children on the diaper changing table.
This follow-up visit inspection recorded no violations or advisories.
A review of five staff records indicated: -Three staff did not have evidence of a current physical examination.
At the time of the visit, there were no staff members present at the program that held a current CPR training certificate. The most recent staff certificates expired in April 2021.
At the time of the visit, there were no staff members present at the program that held a current First Aid training certificate. The most recent staff certificates expired in April 2021. The incident report log had recorded 70 injuries at the program that occurred from April 2021-March 2022. This means, First Aid was preformed by educators who did not hold a current First Aid certification. All educators must obtain within six months of employment, and must maintain thereafter current certification of training in basic first aid appropriate to the population served.

At the time of the visit three children requiring emergency medications were reviewed and the following was found: one child was missing a portion of their prescribed medication.
Safe Sleep
Individual Health Care Plan (IHCP) - R Apr 2025
(REDACTED) child, who had been enrolled in the program for five days, was sleeping in a crib behind a half wall. The child was not in direct visual supervision of a staff member. Children younger than six months of age at the time of enrollment must be under direct visual supervision at all times, including while napping, during the first six weeks they are in care.
A review of five children’s records indicated two Individual Health Care Plans (IHCP) were incomplete.
