At the time of the visit three staff files were reviewed and the following was found: two files did not show evidence of MMRs.
At the time of the visit two children with emergency medications were reviewed and the following was found: both children were missing a portion of their prescribed medication.
Indoor Space
Equipment and supplies: free of entrapment hazards
The wooden fence surrounding the playground was missing a fence panel. This has created an entrapment hazard for children.

A review of the staff records indicated that two educators did not have documentation of two verbal reference verifications.
BRC Policies
A review of the Background Record Check (BRC) Navigator system indicated that seven educator's working at the program were due for renewal. The educators were due for renewals on 9/19/2022, 9/20/2022, 9/24/2022, 9/25/2022, and 9/26/2022.
A review of four Individual Health Care Plans (IHCP) indicated two were incomplete. This caused a child enrolled at the program to not have what would be needed during a potential emergency situation.

Emergency Evacuation - R Apr 2025
During the visit, an educator stated that the emergency medications are stored in the office because staff would forget to return them to the office after bringing them outdoors. Emergency medications such as epinephrine auto-injectors must be immediately available for use as needed.
A review of the evacuation drill log indicated that a practice drill was not held during the month of January 2022. The provider must hold practice evacuation drills with all groups of children and all educators from each floor level of the approved space at least monthly.
A review of four medications indicated four medication consent forms were incomplete. This caused a child enrolled at the program to not have what would be needed during a potential emergency situation.

Individual Health Care Plan (IHCP) - R Apr 2025
The following was observed and accessible to children during the visit: 3's Classroom: -The classroom's garbage container was not covered. 4's Classroom: -An extension cord longer than six inches was hanging off of the wall. -The classroom's garbage container was not covered. 4-day Classroom: -The classroom's garbage container was not covered. -A container of fish food was on the counter in reach of children.
A review of the children's records indicated that two Individual Health Care Plans (IHCP) were incomplete and did not include which educator was trained to address the medical conditions. One of the Individual Health Care Plans (IHCP) was not signed by the parent.
