A review of four children’s records indicated that one Individual Health Care Plan (IHCP) was incomplete.
A review of the evacuation drill log indicated that an emergency evacuation drill was not completed in March 2022 and April 2022. The program must hold practice evacuation drills with all groups of children and all educators from each floor level of the approved space at least monthly.

A review of five children’s records indicated: - One record did not have the child's date of admission or parent signature. - One record did not include evidence of a current physical examination. When discussed, the Director corrected the date of admission.

The depth of the wood mulch in the climber's use zone was approximately 5 inches in depth. The use zone was not covered with an adequate depth of impact absorbing material.
Outside in the play area, on the play house structure and fence there was chipping paint.

A review of five emergency medications indicated: -One medication was not stored in the original packaging with a prescription label.
The following was observed and accessible to children: NURSERY ROOM - The wooden book shelf and white storage unit, under the alphabet bulletin board, were not secure. PRE-K 1 ROOM- Baseboard heating pipes were warm to the touch. PRE-K 2 ROOM- The kitchen structure and white storage unit, next to the room divider, was not secure.
