The program did not have a sufficient depth surface around a two-swing bay, tire swing bay, and a slide. Use zones under and around swings, slides, and climbing structures must be covered with an adequate depth of an impact absorbing material, in accordance with EEC policy. -There is a deck with four individual platforms that appear to be covered in algae, which could pose as a potential slipping hazard for children.

Upon a review of three Staff Records during the visit, the following was identified: -Two records did not have evidence of orientation, EEC Essentials training certificate, physical, evidence of MMR vaccination.
One medication for a child was current but was not stored in its originally dispensed container. The program did however, have the original prescription label affixed to the medication.
Indoor Space
Upon a review of medication stored at the program, there was no(REDACTED) found for a child that was prescribed both an (REDACTED)
Educators have completed the background record check process. However, the BRC Navigator system indicates the Director/Licensee as an "employee" instead of "Licensee."

Medication
The program had a Staff Schedule posted. However, it was not a current Staff Schedule.
Upon a walk through of the facility, the following observations were made: -Next to the door near the loft structure was a trash that was uncovered. -In the bathroom there were plastic hanging bags hanging from a shelf above the toilet that were accessible to children. -In the Art room, there were boxes of assorted ziplock bags, plastic cling wrap in a drawer under the counter that were accessible to children. -The ceiling tile above the Numbers poster and in between the two windows near the bathroom appeared to have water damage stains. -An air conditioning unit in the classroom window had a cord longer than 6 inches accessible to children. -A cd music player located on the small counter to the right of the attendance sheet had a cord longer than 6 inches accessible to children.
A walk through of the playground indicated the following: -There is unknown vegetation growing in several areas on the playground.
A child that was prescribed an (REDACTED) did not have a Medication Consent Form for either medication. This means in the event of a potential emergency, the program is not currently authorized to provide the child life saving medication.
There was no (REDACTED) for a child prescribed this in the event of a potential emergency.
