Indoor Space
A walk through of the outdoor playground area at the time of the visit indicated the following: -The top rail of fencing along the perimeter of the playground (in particular, alongside the tree lined side) was not attached to the portion of the tension wiring of the fence. This means the fencing itself does not meet the minimum height requirements in addition to posing as a potential entrapment issue for children. -There are several areas in which the perimeter of the playground fence posts and spacing between posts appears to be a potential entrapment issue for children. -There are three gaps in the large wooden climbing structure that appear to pose as a potential entrapment issue for children. -There are multiple sections near the rain garden/water area throughout the border of the building in which the bottom portion of the siding of the building has broken and jagged edges that appear to be sharp and can potentially cause injury to a child. -There is a lack of the appropriate depth of safety surfacing around the perimeter of the large climbing structure ("fall zones") as well as the steep hill adjacent to "stairs" on the opposite side of the playground (closer to the building). -There is a lack of adequate depth of sand in the sandbox, exposing the bottom wooden beams of the sandbox which appear to be a potential entrapment hazard to children. -A sandbox cover was not observed to be available. *The outdoor play area must be free from hazards. *All play equipment, fences and structures must be free of entrapment hazards. *6.06 CMR 7.07(16)(e) Playground Safety: The use zones under and around swings, slides, and climbing structures must be covered with an adequate depth of an impact absorbing material, in accordance with EEC policy.

Medication
A walk through of the indoor facilities at the time of the visit indicated the following: -In the "Glass Corridor" hallway, there was exposed drywall and floor tile that was in need of cleaning/repair. The program must monitor the environment daily to ensure that the physical facilities are safe, clean, in good repair.
On the day of the visit, a review of seven medications at the program indicated the following: -One medication prescribed to a child expired in October of 2024. Medications must be appropriately stored, labeled, and not expired.

Upon a review of three Children's Records, -A Medication Consent Form was not signed and dated by the child's parent/guardian for a prescribed (REDACTED) . A Medication Consent Form for (REDACTED) prescribed for the same child was not signed and dated by the parent/guardian. -A Medication Consent Form was not signed by a parent/guardian for a child that was prescribed (REDACTED) .

Upon a review of three Children's Individual Health Care Plans (IHCP), the following was found: -One IHCP was incomplete and did not have the name of the educators listed in the program who were trained in the child's medical condition and the name of the person/agency that trained those individuals. -Two IHCP's were not signed and dated by the child's parent/guardian.
